HID Presenter Information Form

  • This description will be included in the brochure that kids will use to select their classes, so please include enough details to inform their choices.
    The day consists of 5 class sessions of 45 minutes each. Presenters will be scheduled for class sessions based on student demand. Flexibility in scheduling is appreciated.
  • Please enter a number less than or equal to 5.
    A complimentary lunch will be provided to all presenters and volunteers from 11:25 AM - 12:15 PM. Please indicate how many from your party will attend.
  • Please enter a number from 5 to 20.
    Enter the ideal number of students per class. Actual enrollment will depend on student demand and scheduling.
  • This is for any additional supplies you may need to purchase for your class. All reimbursements must be pre-approved by the PTO.
  • These are for materials and supplies not provided by the school but things that you will be bringing into the class. Please denote items for which you will be requesting reimbursement.
  • Please enter a number from 1 to 5.
  • Any foods introduced in the classroom as either part of the presentation or as a treat must be listed here. Please include details and brand names for allergy notification to parents by the school nurse. If you prefer you can attach a PDF file below
